Applicants should forward their applications to the respective Sri Lankan Mission closest to their place of residence.

3. Application Checklist
Applicants are required to submit the following documents:

| 3 | Current Sri Lankan Passport | The current Sri Lankan passport must be submitted. The alteration will be made on the relevant page of the passport. |
| 4 | Supporting Documents | For change of name after marriage: Original Marriage Certificate and a copy. For change of name due to Birth Certificate amendment: Original amended Birth Certificate and a copy. Translations will not be accepted as originals. – Compulsory |

4. Fees

| Within Australia | AUD 17.00 + AUD 10.00 for normal postage |
| Outside Australia | AUD 17.00 + Applicant is responsible for arranged prepaid currier service to return documents. |
Applicants outside Australia, including those from Papua New Guinea, Fiji, Vanuatu, Solomon Islands and Samoa, are responsible for arranging a prepaid courier service for the return of documents.
5. Payment Method
If the applicant visits the High Commission or Consulate General in person, payment can be made by cash or card.
Personal or company cheques will not be accepted.
Applicants who send their application by post should wait for payment instructions. Bank details will be provided by email after the application is received.
Applicants should not send money orders, bank drafts, or directly deposit money into the High Commission or Consulate bank accounts without prior instructions. Any loss, shortage or delay caused by such payments will be the responsibility of the applicant.
6. Important Notes
- Fees will not be refunded under any circumstance after an official receipt has been issued.
- Translations will not be accepted as originals.
- All photocopies must be submitted on A4 size paper.
- The High Commission will attest the documents and return the originals to the applicant.
- If documents need to be sent or received urgently by express courier, speed post or any other similar service, the applicant must arrange this independently.
- The High Commission is unable to take responsibility for documents lost or delayed in the post or courier service.
7. Submission Instructions
Applicants should complete the application form and submit all required documents to the relevant Sri Lankan Mission by post or in person.
Before submitting the application, applicants are advised to ensure that all compulsory documents are included to avoid processing delays.
